A medication used to regulate calcium and phosphate metabolism in patients with disorders caused by impaired vitamin D activation. This helps maintain healthy bones and supports proper calcium levels in the body.

Indications:
- Management of calcium metabolism disorders due to impaired 1-α hydroxylation.
- Treatment of renal osteodystrophy.
- Prevention and treatment of hypoparathyroidism.
- Treatment of neonatal hypocalcemia.
- Management of nutritional and malabsorptive rickets and osteomalacia.
- Management of pseudo-deficiency (D-dependent) rickets and osteomalacia.
- Treatment of hypophosphatemic vitamin D-resistant rickets and osteomalacia.

Main Active Ingredient:
- Alfacalcidol 2 mcg/mL.

Precautions:
- Regular monitoring of calcium, phosphate, and vitamin D levels is essential.
- Use with caution in patients with cardiovascular or renal conditions.
- Avoid overdose to prevent hypercalcemia.

Side Effects:
- Nausea.
- Fatigue.
- Dizziness.
- Itching.
- Hypercalcemia.

Drug-Drug Interactions:
- Increased risk of hypercalcemia with other vitamin D or calcium supplements.
- May interact with digoxin.
- Monitor closely with diuretics.

Initial Dose:
- Adults and children >20 kg: 1 microgram/day.
- Elderly: 0.5 microgram/day.
- Neonates and premature infants: 0.05–0.1 microgram/kg/day.
- Children <20 kg: 0.05 microgram/kg/day.

Dosing Notes:
- Half-drop doses should be rounded up to the nearest whole drop.
- Plasma levels should be monitored weekly at the start of treatment.

Dose Adjustment:
- Daily dose may be increased in increments of 0.25–0.5 microgram.
- Once stabilized, plasma measurements may be performed every 2–4 weeks.

Typical Adult Response:
- Most adults respond to doses between 1 and 3 micrograms per day.
- When biochemical or radiographic evidence of bone healing is observed, or in hypoparathyroid patients once plasma calcium levels normalize, the dose is usually reduced.

Maintenance Dose:
- Typically ranges from 0.25 to 1 microgram per day.
